TogetherNow is a nonprofit social care enterprise working to transform how people access services across healthcare, education, human services and the public sector. Our mission is to remove systemic barriers and help individuals receive coordinated, dignified support — without having to become experts in fragmented systems. TogetherNow’s model combines digital infrastructure and human-centered design to create inclusive, accountable systems that work with people, not on them.

Our flagship tool, MyWayfinder – https://my-wayfinder.org/home, helps people find and connect with services, track their needs, and build a care team of trusted providers. Beyond the technology, we lead cross-sector collaboratives, facilitate workflow redesign with frontline staff, and teach others in our community how to use Human-Centered Design (HCD) to improve service delivery.

TogetherNow advances SDG 16 (Peace, Justice and Strong Institutions) by making public service systems more human-centered, coordinated, and accessible, particularly for communities affected by poverty and systemic inequities.
